Mentee Referral

Please, complete this form if you have experienced homelessness or are at risk of becoming homeless and would like to be considered for mentoring

or 

you are a professional, friend or relation supporting someone who has had experienced the above and you would like them to be considered for mentoring.

You may call me if the person I am referring does not have a telephone number

  • can cope with and would benefit from 3-6 months of Face to Face or Telephone Mentoring

  • are 18 or over

  • has given me consent to make this referral on their behalf

All referrals will be contacted by the Mentor Coordinator in the next seven working days.
